Please Scroll down to the bottom of the page and use your booking reference number to upload documents.
For Birth Registration Uploads
The birth card issued by the midwife or hospital showing the date and time of the baby's birth.
Parent's marriage/civil partnership certificate (if applicable).
For Death Registration Uploads
Any birth, death, marriage/civil partnership or divorce certificates related to the deceased where available (e.g. parent's, spouse etc).
If you have selected to use the Tell Us Once Service we would require copies of the Passport, Driver's Licence, Blue Badge, Bus Pass, National Insurance Number where available.
For Marriage / Civil Partnership Uploads
M10 or CP10 notice forms must not be uploaded - only signed hard copy can be submitted either in person or by post.
Amendments to ceremony wording must be uploaded to your booking no later than 2 weeks before your ceremony.
