There is an option for your appointment to take place in person at one of our registration offices or by phone.
Please have the hospital/midwife birth registration card ready for your appointment.
You will receive a telephone call from our team at the appointment time you choose.
The registration will be carried out remotely. Please have any birth certificates etc to hand at the time of the appointment.
Upload Documents to Existing Booking
Attach documents to your booking, such as evidence for using the Tell us Once Service as part of a Death registration.
You can also upload civil ceremony content and completed requirements form.
